Fees apply to uninsured services. These are services that the Alberta government does not pay doctors to perform. In these cases, patients pay ‘out of pocket’. We will provide you with a receipt.

General Fees

  • Sick or off work notes – $75
  • Prescription for allied health (physiotherapy, massage therapy, chiropractic, acupuncture, etc.) – $75
  • Special authorization forms for medications – $65 – $250
  • Supplemental benefits top up form – $125
  • Insurance and Disability Forms – $325-$550 
  • Missed appointment- $300+
  • Uninsured office visits – prices vary
  • Transfer of chart to another physician’s office – $60+
  • Copy of medical records to a third party, medico-legal services – prices vary

Please Note

There may be additional charges based on:

  • Additional time to complete forms or preparation time at $500/hr+
  • Rush services at 2x base service cost
