Fees apply to uninsured services. These are services that the ___ does not pay doctors to perform. In these cases, patients pay ‘out of pocket’. We will provide you with a receipt.

General Fees

  • Sick or off work notes – $55
  • Prescription for allied health (physiotherapy, massage therapy, chiropractic, acupuncture, etc.) – $55
  • Special authorization forms for medications – $55 – $200
  • Supplemental benefits top up form – $105
  • Insurance and Disability Forms – $250-$500 
  • Missed appointment- $300+
  • Uninsured office visits – prices vary
  • Transfer of chart to another physician’s office – $25+
  • Copy of medical records to a third party , medico-legal services – prices vary

There may be additional charges based on:

  • Additional time to complete forms or preparation time at $500/hr+
  • Rush services at 2x base service cost
